• PA Navigate is an online tool from PA DHS that allows users to search for and connect to medical and social supports by zip code, including community-based organizations, county and state agencies, and health care providers. Through the tool, you can refer yourself or others to the supports they need to facilitate connection and communication. It's a great starting point for quickly connecting struggling patients to the supports they need.
