The survey takes no more than 15 minutes and will guide continuing education programs and practice supports and support lobbying efforts with the state and insurance companies who could better assist us with this growing public health problem. We appreciate your effort on this project and will provide feedback through your state professional organization.

BY COMPLETING THIS SURVEY, YOU GIVE US PERMISSION TO USE YOUR RESPONSES IN A RESEARCH PROJECT. This survey is part of a research study being conducted at the Children's Hospital of Philadelphia. Your participation is fully voluntary. The survey is anonymous and will not collect any information which could identify you. Data will be aggregated and only examined as a group. No one will be able to link this data back to you as an individual. Thus, there is minimal risk of loss of confidentiality if you agree to participate. In addition, the benefits to the larger primary care community could be important. The study data will be used to inform policy makers and payers about the needs for more support for mental health issues in primary care. Your alternative to completing this survey is to not complete the survey. If you decide not to participate or if you change your mind, there will be no penalties or loss of benefits to which you are otherwise entitled.
